Athletic Participation

As a member, FHSAA requires that the same paperwork be completed for every CCS Middle School and High School Athlete. Each athlete must have a current Physical Form (EL2) and the Consent Form (EL3) on file through Final Forms.

For PEP & homeschooling athletes, additional forms are required. See the Final Forms link for more information.

Final Forms is the program we are using for our athletic paperwork. The program allows parents to register students, upload forms into the system and roll over the information from year to year. This program ensures paperwork isn’t misplaced and parents can keep the originals. Please do not turn in FHSAA paperwork to the school or the coaches.

NEW FOR 2026 FALL CCS CROSS COUNTRY (or any CCS Sport):

If you are an incoming freshman (9th grader) or are a high school student who has NOT participated in interscholastic sports anywhere since 9th grade, you will need an EKG Screening completed to participate in any 2026/2027 CCS Sport; this is a new FHSAA requirement. The organization, Who We Play For is doing EKGs for Free (or a low cost). Visit this link, https://www.whoweplayfor.org/heart-screening-registration for more information and for times/locations of clinics. This EKG Screening requirement goes into effect on July 1, 2026; only one EKG screening/passing is required for participation in CCS Sports for the length of your Athlete's FHSAA participation eligibility. Download this form and take it to your appointment.

Please click on the Final Forms link to begin the process. Final Forms For assistance with the registration process, please click here

All athletes must have the required FHSAA Paperwork submitted prior to a student-athlete being eligible to participate in conditioning, practices or games.

Cost per sport is $185 for full-time CCS students.   The fee for CEO, PEP & Homeschool students is $350 per sport.
